Use of checklists for identification

A checklist approach is often used, such as the one below:

Social

poor attendance and/or punctuality;

frequent moves of school;

nursery attendance;

socialisation problems;

chooses younger children to play with or prefers playing by themself.

Physical

visual — near- or far-sighted, wearing inappropriate glasses, failure to wear glasses, seated in poor position to see;

auditory — temporary (glue ear), permanent — with or without hearing aid;

kinaesthetic — mobility, medical conditions, including those involving medication that may make the child drowsy.
